• If you are not sure which doctor you would like to see, please call us at (903) 614-3000.
• Remember that not all doctors are taking online appointment requests.
• If you are needing an appointment within 48 hours, please call us. Otherwise complete the following form and you will be contacted by a clinic representative to confirm an appointment date and time. The information you provide on this page should be secure and confidential. If you do not see a lock or key on your browser, indicating a secure connection to our server, please submit your request via phone.
• If you are looking for a new primary care physician at the main clinic or at one of the other facilities, please call 903.614.3000
• If you are currently under a physician's care at the clinic, call your physician's department directly for appointment information
• Doctors listed in blue do not take online appointment requests.
• All appointment requests made after 5:00pm during the M-F work week will be processed on the following work day. All requests made on Saturday and Sunday will be processed on the next workday.
